Lunch and Learn: A Day in the Life of a Care Coordinator
Hosted by Nationwide Children’s Hospital and Partners For Kids, this CEU lunch and learn will provide insight into what care coordination is and how all disciplines within the care team can work together to improve patient outcomes.
The desired outcome for attendees will be to develop an understanding of the care coordination scope of practice, increase inter-disciplinary collaboration skills, understand how care coordination impacts patient care, and learn how to assist patients and families with transitioning towards self-management and family-centered care.
