Partners for Kids accepts referrals for pregnant teens and all newborn babies, regardless of risk, into Care Navigation. Available services include obtaining necessary resources for mother and infant, Medicaid enrollment for the infant, scheduling infant well-child visits, and other services to address social determinants of health. If you have identified a pregnant teen or newborn who has an Ohio Medicaid Managed Care plan, please consider referring them to Partners For Kids’ Care Navigation using one of the following ways:
