Potential Exposure of Ohio Medicaid Provider Information by ODM Vendor

Maximus, a subcontractor of the Ohio Department of Medicaid (ODM), experienced a data breach in May involving the personal information of over 330,000 Medicaid providers. Maximus has started notifying impacted individuals via U.S. mail. This issue is unrelated to Partners For Kids’ systems. Any questions related to the nature of the breach should be directed to Maximus via the contact information provided in the notice to impacted providers.

Here are resources and suggestions for impacted providers.

  • Visit the Ohio Attorney General for free resources and information on identity theft.
  • Consider enrolling in the complimentary credit monitoring service offered by Maximus.
  • Set up a free fraud alert with the credit reporting agencies (Experian, Equifax, TransUnion). You only need to do this with one agency, as all agencies will be notified of the fraud alert.
  • Consider freezing your credit report. This prevents anyone from doing a credit check without your authorization. Credit freezes must be applied for at each credit reporting agency (Experian, Equifax, TransUnion).
