Upcoming Lunch and Learn: The Patient Experience of Transitioning from Inpatient to Outpatient Care
As a nurse or social worker, have you ever wondered what happens to patients with medical complexities after they leave the hospital?
The period following discharge from the hospital is a vulnerable time for our pediatric patients. Challenges can include changing providers, medication changes, new self-care responsibilities, which generate a need for resources and an understanding of complex discharge instructions.
Come learn about the family’s experience, common problems and successful strategies that generate best outcomes for children and their families. We will compare and contrast three pilot programs that have been implemented at Nationwide Children’s Hospital, each using a different method to support patients and providers during this time of transition.
