Ways to Engage in Patient Outreach with Partners For Kids

There are several ways Partners For Kids can help your office with outreach efforts! Each of these is at no cost to your office and can assist with any efforts already being done. Our team will work alongside you to create an outreach plan and materials that best fit your needs and patients. Some examples include; 

Sending customized postcards to remind patients to call and schedule their appointment.    

  • Monthly mail out for patients due or overdue for their well care visit  
  • Cross check of EMR to ensure patients have not already been scheduled before sending  
  • Report out on success rate of completed visits after outreach 

Customizable tear off sheets:  

  • Immunization series information and next dose date  
  • Extended hours announcement  
  • Benefits of well-child visit vs. Physical at a walk-in clinic  
  • Flu shot clinic dates
