PFK Asthma Risk Portal: Identify and Learn About Your High-Risk Asthma Patients

Do you ever find yourself wondering which of your asthma patients are most likely to have a future exacerbation, and what exacerbation events are occurring outside of your office? If so, Partners For Kids has developed a tool for you!

In collaboration with the Nationwide Children’s Hospital Data Science team, Partners For Kids developed a risk model to predict which asthma patients have the highest chance of a future emergency department or inpatient asthma event. This risk model was then incorporated into the Asthma Risk Portal, and it is located on the Partners For Kids Provider Portal. The Asthma Risk Portal outlines who each practice’s high-risk patients are and provides an automated 12-month claims history for each patient, detailing medication and exacerbation events that placed them at high risk.

To maximize this resource, the Partners For Kids team created a tutorial that details how to access the Asthma Risk Portal and outlines best practices for implementation of this data into your practice.  For questions about the Asthma Risk Portal, including scheduling an education session with the Partners For Kids Asthma Team, please reach out to your provider relations representative at PFKProRelations@NationwideChildrens.org.
