Celebrating 10 Years of Partners For Kids Care Coordination

This October, Partners For Kids recognized the 10th anniversary of its delegated care coordination program for Ohio’s children and families. Over the last decade, PFK Care Coordination has achieved extraordinary things on behalf of its members, including: 

  • Improving the coordination of care for over 40,000 children and their families 
  • Reducing unnecessary utilization of the ED and inpatient hospitalization for multiple consecutive years 
  • Improving integrated care according to the patient perspective 
  • Leading rates of outreach success and successful completion of care coordination goals 
  • Establishment of innovative methods of coordinating care, like televisits, and use of the electronic record to communicate with patients and their care team 
  • State of the art orientation and training curriculum for nurses, social workers, leaders and ancillary staff who are newly performing a care coordination role 

Partners For Kids created a video capturing the reflections of key leaders to celebrate these landmark achievements, as seen here. 

 To further recognize this occasion, Partners For Kids staff assembled a collection of posters summarizing the department’s quality improvement work and key successes. Poster topics included: 

  • Impact of Partners For Kids Care Navigation on Justice-Involved Youth 
  • Effectiveness of Partners For Kids Care Navigation efforts to increase SSI take-up 
  • Efficacy of PFK Care Navigation interventions in post-discharge NICU population 
  • Case study exhibiting the benefits of Partners For Kids Care Navigation for a medically and behaviorally complex child and that child’s family 
  • Impact of a Partners For Kids transition-to-practice program for pediatric case management 
  • Examination of Partners For Kids Care Navigation’s role in reducing child maltreatment 
  • Comparative evaluation of the impact of length of enrollment in Partners For Kids Care Navigation on case outcomes for high-risk patients. 
  • Effectiveness of Partners For Kids Care Guide program in its first year of operation 
  • Effectiveness of Partners For Kids Health Risk Assessment team 

Partners For Kids hosted an Executive Happy Hour on October 10th, inviting collaborators and colleagues from the Ohio Department of Medicaid, other pediatric ACOs, and partners among Ohio’s managed care organizations, Dayton Children’s Hospital and Nationwide Children’s Hospital to view the posters and visit with families who have been impacted by the program. 

Partners For Kids is proud to celebrate more than a decade of best outcomes for Ohio’s children through its care coordination efforts and would like to thank our tremendous network of providers for their partnership in reaching this milestone.
