Healthy Children Toolkit for Primary Care

 

This Partners For Kids toolkit is designed to assist primary care practices and providers when providing preventive care services including well child visits, immunizations, and lead screening. Below is a curated list of items including example process maps, educational materials, resources, and helpful links. This is not intended to be an exhaustive list of all preventive care resources available. The information housed in this toolkit will be updated as needed based on current clinical guidelines and recommendations.
