Did you know Partners For Kids now offers a Care Guide team for your patient families who are enrolled in Medicaid or could be eligible for Medicaid? The team offers expert staff trained to help families with a variety of issues related to social determinants of health, including Medicaid enrollment, connection to food assistance and housing, coordinating transportation, scheduling appointments, finding affordable childcare and more. The team can assist on a short-term basis (up to 60 days) when there is a positive health risk assessment screening.
