Partners For Kids is proud to announce the launch of our newest quality improvement project titled “Constipation Management in Primary Care”. This project was created for primary care providers to enhance the diagnosis, treatment, and follow-up care for functional constipation. The Partners for Kids’ Quality Improvement (QI) Coaching team will collaborate with your practice to customize and enhance constipation management for pediatric patients. Key areas of focus include:
- Medication Management: Optimize the use of first-line treatments and improve adherence to prescribed regimens to ensure effective symptom relief.
- Patient & Family Education: Provide comprehensive, easy-to-understand educational materials for families, helping them manage constipation at home and helping them understand when to seek medical care.
- Follow-up Care & Monitoring: Implement a structured follow-up system to monitor treatment progress and adjust care plans as needed, ensuring ongoing patient support.
- Care Coordination: Integrate care coordination services to facilitate communication between providers and ensure seamless transitions of care, especially for high-risk patients or those with complex needs.
- Minimization of Unnecessary Referrals: Reduce the need for specialist referrals and unnecessary diagnostic tests through improved initial management and early intervention strategies.
Maintenance of Certification Part IV is offered to community providers who participate in this quality improvement (QI) project.
