The Partners For Kids Provider Portal’s mismatched attribution report shows a list of patients who were seen for a well–care visit at your practice, but who are not attributed to the provider completing the visit. This report now includes a link to the PCP Change Request Form. If a family wants to change their attributed provider, they can complete the PCP Change Request Form and send the updated form to the patient’s assigned Managed Care Plan. If you have questions about the Provider Portal, please contact PFKProRelations@NationwideChildrens.org if you are located in the Central Region and PFKProRelations@ChildrensDayton.org if you are located in the West Region.
