The Partners For Kids Provider Portal’s mismatched attribution report shows a list of patients who were seen for a well–care visit at your practice, but who are not attributed to the provider completing the visit. This report now includes a link to the PCP Change Request Form. If a family wants to change their attributed provider, they can complete the PCP Change Request Form and send the updated form to the patient’s assigned Managed Care Plan.
