Providers can use the PartnersForKids.org website to refer patients to Partners For Kids Care Coordination. You can either go to PartnersForKids.org, click on the three horizontal lines in the top left corner and choose “Contact us” or bookmark the “Contact Us” page. Once there, under “Topic,” you can select “Referring a patient for Care Coordination services.”
