Did You Know? – PCP Change Request Form

The Primary Care Provider (PCP) change request form can be used to update the provider to whom a patient is attributed. The attribution process is fully handled through the managed care plans. However, patient choice is the first criterion that determines which provider the patient should be attributed. To ensure providers are receiving credit for their patients, the provider’s name must appear on the back of the patient’s Medicaid card. To update your patient’s Medicaid card, have the patient sign the PCP change request form and fax it to the associated Medicaid plan.

Partners For Kids has also made it easier to identify the kids needing to complete this process. On the new Partners For Kids provider portal, you will find a mismatched attribution report. This report identifies patients that receive care from your providers but are not attributed to your practice. Reach out to your Partners For Kids provider relations specialist if you would like to obtain access to the provider portal to review this report.
