Foundations Behavioral Health Services is a cornerstone of community-based behavioral health care, proudly serving Mercer, Van Wert and Paulding Counties, along with the surrounding areas. They provide mental health, substance use services and medication services using case managers and therapists, as well as staffing a psychiatrist and nurse practitioner.
