Partners For Kids has developed an RSV toolkit where you will find communication from Partners For Kids on RSV, nirsevimab updates, materials that help explain the importance of RSV prevention, and recommendations for using nirsevimab to patients.
Comprehensive Primary Care (CPC) Toolkit
The comprehensive primary care (CPC) toolkit has been developed to assist primary care practices and providers in navigating the CPC program. The toolkit highlights a curated list of items, including a CPC overview, enrollment information, an overview of activity metrics, and how Partners For Kids can support practices in the CPC program.
Healthy Children Toolkit for Primary Care – West Region
This toolkit from PFK provides information and resources on the provision of preventive care services including well care visits, immunizations, and lead screening.
Electronic WIC / SNAP Tear-Off Pads
Partners For Kids has developed marketing materials to share with patients that may meet the eligibility criteria for the SNAP and WIC programs. Patients receiving these benefits will automatically be re-enrolled in Medicaid during the redetermination process.
West Central Region Materials for Medicaid Enrollment
Every person covered by Medicaid must have their eligibility determined annually, but that process has been suspended during the federal public health emergency of COVID-19. It is now expected that the public health emergency will end in mid-April 2023, and the redetermination process will resume. Providers have a role to play in ensuring your patients stay covered.
