Partners For Kids’ care navigation team partners with patients and families to help navigate the healthcare system and community services. Interventions can be as big as coordinating a care conference for a complex medical child with many specialty needs, or as small as working with a healthy child that needs to be connected to a dental provider. There is no barrier too big or too small. If you have a patient that you think could benefit from care navigation, you can make a referral the following ways:
